Anticipating your first massage with a new therapist may be stressful if you don't know what to expect. Since my goal is to help reduce your stress, I want you to know as much as possible about what to expect at your first session. So if you have any questions that aren't answered here or on the FAQ page, please don't hesitate to contact me.

Paperwork

When beginning treatment with a new therapist, it is important to provide them with as much information about your health and context as possible. To that end, please bring any relevant health history and medical records with you to your first session, or please be sure to share all medical history in your intake paperwork.

At your first session you will need to fill out some intake paperwork. If you would like to complete your intake form prior to your appointment, you may download and print the New Client Intake Form and take your time filling it out at home. However, while this is helpful and time-saving, it is not required, since your new patient appointment includes time for paperwork. Regardless of when you complete the paperwork, the more thorough you are, the more able I will be to provide you with the highest quality care.

Arrival & Apparel

You do not need to arrive early; your new patient appointment allows time to complete all paperwork and the initial consultation before your full massage.

If you would prefer to stay clothed during your massage, please dress comfortably. Because I use a high-quality cream, there is no need to worry about the effects of oil on your clothes.

Treatment of Minors

If you are under the age of 18, you must bring with you a Consent to Treat Form signed by a parent or legal guardian.

Submitting to Insurance

If you are submitting your session to insurance, are under supervised medical care, or would like to share your doctor's or therapist's treatment plan with me, please also bring a completed Physician Referral Form with you to your appointment.
